VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT Last Updated: 02/01/22 Page 1 of 1 DEPARTMENT OF VETERANS AFFAIRS Justification for Single Source Awards IAW FAR 13.106-1 for Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K) Acquisition Plan Action ID: 36C252-22-AP-2435 Contracting Activity: Department of Veterans Affairs Great Lakes Acquisition Center, NCO-12 115 S. 84th Street, Suite 101 Milwaukee, WI 53214-1476 William S Middleton Memorial VAH 2500 Overlook Terrace Madison, WI 53705 607-22-3-969-0147 Brief description of supplies/services required and the intended use/estimated amount: The William S Middleton Memorial VAH in Madison, Wisconsin has a need for the purchase and delivery of a ACIST CVi Standard Contrast Delivery System and accessory items to be utilized by the VAH Cardiac Catheterization lab. Estimated Amount: [REDACTED] Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Cardiac Catheterization lab requires a way to access the functionality of significant coronary artery lesions prior to an intervention procedure. Currently their (non-interventional) catheterization physicians have no way of evaluating these lesions prior to enlisting the help of the interventionalists. This can cause significant delays during which the patient is on the procedure table with a catheter in an artery. The current catheterization lab x-ray system is no longer supported by the manufacturer for software upgrades. Most of the vendors of the required type of system have software/hardware as part of their x-ray systems and do not manufacture it for their competitors. The ACIST CVi Standard Contrast Delivery System would allow the catheterization physicians to evaluate the disease and determine whether a percutaneous intervention (PCI) is an option or if the patient should be sent to surgery or medical management. If a patient was found to not have lesions amenable to PCI, they would not need to wait for the Interventionalist to arrive to make the determination for treatment. Description of market research conducted and results or statement why it was not conducted: VIP search was conducted using the search criteria of NAICS Code: 339112 and the Keywords: contrast delivery and no organizations were found. VIP search was conducted using the search criteria of NAICS Code: 339112 and the Keywords: contrast injector and 2 SDVOSB organizations were found [REDACTED] and and [REDACTED]. Both organizations listed maintenance and repair in their capabilities statements, but not manufacturing of contrast delivery systems. Neither are authorized providers of the ACIST CVi contrast injectors. B2H provided a Letter of Authorization by ACIST Medical Systems, Inc. appointing B2H as an authorized distributor of all ACIST products to the U.S. Department of Defense and Veteran s Administration facilities in the United States and its territories. B2H verified that the product is manufactured in the United States. An Intent to Sole Source was issued via SAM on 9/15/2022 with a response date of 9/19/2022; amended on 9/16/2022 to change the cite referenced from FAR 13.106-1(b) to VAAR 819.7007 and extending the response date from 9/19/2022 to 9/20/2022. One response was received on 9/15/2022 from [REDACTED]. His entity is not registered in Vetbiz and specializes in window washing. He has been trying to get registered in Vetbiz but has been unsuccessful. His business information was forwarded to our Small Business Liaison to further assist with his Vetbiz registration. Based on the information provided above, it is determined this procurement will be fulfilled by issuing a sole source Purchase Order to B2H Consulting Group, LLC (UEI: C3CDMJFSLAY9), an SDVOSB in accordance with 38 U.S.C. 8127(b) and VAAR 819.7007. Contracting Officer's Certification: Purchase is approved in accordance with FAR 13.106-1(b) and VAAR 819.7007.
