Background
The Department of Veterans Affairs (VA) is issuing this solicitation to establish a supply source for Zonisamide Capsules through its Pharmaceutical Prime Vendor (PPV) Programs. The goal is to ensure availability and consistency of pharmaceutical products for nationwide usage while obtaining volume-based, committed-use pricing. This contract will provide drugs listed in the schedule for purchase through the PPV Programs, which include various government health facilities.

Work Details
The contract involves the delivery of Zonisamide Capsules in three different dosages, with specific quantities and packaging requirements as follows:

1. **Zonisamide 100mg Capsules**
- Quantity: 41,351 Bottles per year for Base Year and each Option Year (1-4)
- Packaging: 100 Tablets per Bottle
- NDC # and ANDA# required

2. **Zonisamide 25mg Capsules**
- Quantity: 4,539 Bottles per year for Base Year and each Option Year (1-4)
- Packaging: 100 Tablets per Bottle
- NDC # and ANDA# required

3. **Zonisamide 50mg Capsules**
- Quantity: 5,326 Bottles per year for Base Year and each Option Year (1-4)
- Packaging: 100 Tablets per Bottle
- NDC # and ANDA# required.

Offerors must submit prices that include a Cost Recovery Fee of 0.5% and provide an 11-digit NDC number unique to their company for each drug offered.

Period of Performance
The contract will be effective upon signing by the VA and will include a base year followed by four one-year options.

Place of Performance
Products will be delivered to various Department of Veterans Affairs facilities, Department of Defense facilities, Indian Health Service facilities, and Bureau of Prisons facilities across the United States.
