Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for pharmaceutical products through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of Phenytoin Sodium Extended 100MG Sustained Action Capsules for nationwide usage, while obtaining volume-based pricing for various government health facilities including VA, DoD, IHS, and BOP.

Work Details
The contract involves the supply of Phenytoin Sodium Extended 100MG Sustained Action Capsules in two packaging sizes: 100 count bottles and 1,000 count bottles. The quantities required are as follows:

- **Line Item 1**: Phenytoin NA Extended 100MG SA Capsules, 100 count bottles
- Base Year: 8,470 Bottles
- Option Year One: 8,470 Bottles
- Option Year Two: 8,470 Bottles
- Option Year Three: 8,470 Bottles
- Option Year Four: 8,470 Bottles

- **Line Item 2**: Phenytoin NA Extended 100MG SA Capsules, 1,000 count bottles
- Base Year: 8,183 Bottles
- Option Year One: 8,183 Bottles
- Option Year Two: 8,183 Bottles
- Option Year Three: 8,183 Bottles
- Option Year Four: 8,183 Bottles.

Offerors must provide a unique National Drug Code (NDC) number for each offered drug and include a Cost Recovery Fee of 0.5% in their pricing. The contract will be awarded based on the aggregate price for all line items across the base year and four option years.

Period of Performance
The contract will be effective upon signing by the VA and notification to the contractor. The implementation period will last up to a maximum of 60 calendar days before the ordering period begins.

Place of Performance
The products will be delivered to various Department of Veterans Affairs facilities and other authorized users under the Department of Defense Pharmaceutical Prime Vendor Program.
