Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Perphenazine Tablets through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of pharmaceutical products for nationwide usage while obtaining volume-based pricing.

This solicitation aims to provide drugs that will be distributed through the PPV Programs, which are essential for various government health facilities including VA, DoD, IHS, and BOP.

Work Details
The contract includes the following items:
1. **Perphenazine 2MG Tablets**
- Quantity: 3,466 Bottles per year for Base Year and each Option Year.
2. **Perphenazine 4MG Tablets**
- Quantity: 5,191 Bottles per year for Base Year and each Option Year.
3. **Perphenazine 8MG Tablets**
- Quantity: 3,615 Bottles per year for Base Year and each Option Year.
4. **Perphenazine 16MG Tablets**
- Quantity: 1,137 Bottles per year for Base Year and each Option Year.

Each offer must include an 11-digit National Drug Code (NDC) number unique to the offeror's company, and prices must include a Cost Recovery Fee of 0.5%.

Period of Performance
The contract will be effective upon signing by the authorized representative of the VA and will include a base year with four one-year options.

Place of Performance
Products will be delivered to various locations as specified by the Department of Veterans Affairs and other authorized users under the PPV Programs.
