The contract involves the supply of Nicotine Polacrilex Mini Lozenges in two strengths: 2mg and 4mg.

Line Item 1 requires 113,911 cartons of 2mg lozenges per year, while Line Item 2 requires 94,418 cartons of 4mg lozenges per year. Offerors must submit prices for a base year and four option years, including a Cost Recovery Fee of 0.5%. Each offered drug must have a unique 11-digit NDC/UPC number.
