Background
The Department of Veterans Affairs (VA) is issuing this solicitation to establish a supply source for pharmaceutical products through the VA and Department of Defense (DoD) Pharmaceutical Prime Vendor Programs. The goal is to ensure availability and consistency of nicotine patches for nationwide usage, while obtaining volume-based, committed-use pricing. This contract will facilitate the distribution of nicotine patches to various government health facilities.

The contract includes the following items:

1. NICOTINE 14MG/24HRS PATCH, 14 Patches per Box
- Base Year: 241,043 Boxes
- Option Year One: 241,043 Boxes
- Option Year Two: 241,043 Boxes
- Option Year Three: 241,043 Boxes
- Option Year Four: 241,043 Boxes

2. NICOTINE 21MG/24HRS PATCH, 14 Patches per Box
- Base Year: 315,451 Boxes
- Option Year One: 315,451 Boxes
- Option Year Two: 315,451 Boxes
- Option Year Three: 315,451 Boxes
- Option Year Four: 315,451 Boxes

3. NICOTINE 7MG/24HRS PATCH, 14 Patches per Box
- Base Year: 147,215 Boxes
- Option Year One: 147,215 Boxes
- Option Year Two: 147,215 Boxes
- Option Year Three: 147,215 Boxes
- Option Year Four: 147,215 Boxes.

Offerors must provide a unique NDC number for each drug offered and include a Cost Recovery Fee of 0.5%.

Period of Performance
The contract will be effective on the date the VA signs the SF-1449 and will include a base year with four one-year options.

Place of Performance
Products will be delivered to various Department of Veterans Affairs facilities and other authorized users under the DoD Pharmaceutical Prime Vendor Program.
