Mycophenolate Mofetil RFP   2

Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Mycophenolate Mofetil, a pharmaceutical product, through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of the specified drugs for nationwide usage while obtaining volume-based pricing. The contract will serve all VA facilities, Department of Defense (DoD) facilities, Indian Health Service (IHS) facilities, and Bureau of Prisons (BOP) facilities.

Work Details
The contract includes the following items:

1. **Mycophenolate Mofetil 250mg Capsules**
- Quantity: 18,706 Bottles per year for Base Year and each Option Year
- NDC # and ANDA# required

2. **Mycophenolate Mofetil 250mg Capsules** (500 Capsules per Bottle)
- Quantity: 17,378 Bottles per year for Base Year and each Option Year
- NDC # and ANDA# required

3. **Mycophenolate Mofetil 500mg Tablets**
- Quantity: 7,307 Bottles per year for Base Year and each Option Year
- NDC # and ANDA# required

4. **Mycophenolate Mofetil 500mg Tablets** (500 Tablets per Bottle)
- Quantity: 18,281 Bottles per year for Base Year and each Option Year
- NDC # and ANDA# required.

Offerors must submit prices that include a Cost Recovery Fee of 0.5% and provide an 11-digit NDC number unique to their company for each offered drug.

Period of Performance
The contract will be effective upon signing by the VA and will include a base year with four one-year options.

Place of Performance
Products will be delivered to various locations as specified by the VA's PPV Programs.
