Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Montelukast Chewable Tablets through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of pharmaceutical products for nationwide usage, while obtaining volume-based, committed-use pricing. This solicitation aims to provide drugs listed in the schedule for purchase through the PPV Programs, which include various government health facilities.

Work Details
The contract involves the delivery of Montelukast Sodium Chewable Tablets in various packaging sizes. The specific line items include:

1. **Montelukast NA 4MG Tablets, Chewable**
- Quantity: 2,625 Bottles per year for Base Year and each Option Year (Total: 10,500 Bottles)
- Packaging: 30 Tablets per Bottle

2. **Montelukast NA 4MG Tablets, Chewable**
- Quantity: 5,988 Bottles per year for Base Year and each Option Year (Total: 23,952 Bottles)
- Packaging: 90 Tablets per Bottle

3. **Montelukast NA 5MG Tablets, Chewable**
- Quantity: 4,532 Bottles per year for Base Year and each Option Year (Total: 18,128 Bottles)
- Packaging: 30 Tablets per Bottle

4. **Montelukast NA 5MG Tablets, Chewable**
- Quantity: 15,887 Bottles per year for Base Year and each Option Year (Total: 63,548 Bottles)
- Packaging: 90 Tablets per Bottle

5. **Montelukast NA 5MG Tablets, Chewable**
- Quantity: 202 Bottles per year for Base Year and each Option Year (Total: 808 Bottles)
- Packaging: 1,000 Tablets per Bottle.

Each bottle must have a safety-cap design that allows for easy opening while maintaining safety standards. Offerors must provide an NDC number unique to their company for each drug offered.

Period of Performance
The contract will be effective upon signing and notification of award by the VA. The Implementation Period will last up to a maximum of 60 days post-award before the Ordering Period begins.

Place of Performance
Products will be delivered to various Department of Veterans Affairs facilities as well as Department of Defense facilities participating in the Pharmaceutical Prime Vendor Program.
