The goal of this contract is to ensure the availability and consistency of pharmaceutical products for nationwide usage, while obtaining volume-based pricing. This solicitation aims to provide a reliable source for drugs required by various government health care facilities, including the VA, Department of Defense (DoD), and other federal agencies.

- **Base Year**: 97,242 Bottles
- **Option Year One**: 97,242 Bottles
- **Option Year Two**: 97,242 Bottles
- **Option Year Three**: 97,242 Bottles
- **Option Year Four**: 97,242 Bottles

2. **Line Item 2a** - Metformin HCL IR Tablets, 1000mg Tablets, 500 Tablets per Bottle
- **Base Year**: 354,062 Bottles
- **Option Year One**: 354,062 Bottles
- **Option Year Two**: 354,062 Bottles
- **Option Year Three**: 354,062 Bottles
- **Option Year Four**: 354,062 Bottles

3. **Line Item 2b** - Metformin HCL IR Tablets, 1000mg Tablets, 1000 Tablets per Bottle
- Same quantities as Line Item 2a.

4. **Line Item 3** - Metformin HCL IR Tablets, 500mg Tablets, 100 Tablets per Bottle
- **Base Year**: 20,017 Bottles
- **Option Years One to Four**: Same quantity.

5. **Line Item 4a** - Metformin HCL IR Tablets, 500mg Tablets, 500 Tablets per Bottle
- **Base Year**: 307,340 Bottles
- Same quantities for Option Years.

6. **Line Item 4b** - Metformin HCL IR Tablets, 500mg Tablets, 1000 Tablets per Bottle.
7. **Line Item 5** - Metformin HCL IR Tablets, 850mg Tablets, 100 Tablets per Bottle.
8. **Line Item 6a** - Metformin HCL IR Tablets, 850mg Tablets, 500 Tablets per Bottle.
9. **Line Item 6b** - Metformin HCL IR Tablets, 850mg Tablets, 1000 Tablets per Bottle.
Each offer must include an NDC number unique to the Offeror's company and comply with all specifications outlined in the solicitation.

Period of Performance
The contract will be effective upon signing and will include a base year with four one-year options.

Place of Performance
The products will be delivered to multiple PPV distribution centers across the United States as specified in the contract.
