Amendment to Extend Due Date and Time for Offers This amendment is issued to extend the due date and time for receipt of offers submitted in response to solicitation 36E79725R0051 for Metformin Hydrochloride (HCL) 24HR Sustained Action (SA) Tablets from 8/11/2025 to 8/25/2025 at 2:30PM Central Time. All other solicitation terms and conditions remain unchanged. Offerors shall include a completed and signed copy of the amendment with their proposal.

Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Metformin Hydrochloride (HCL) 24HR Sustained Action (SA) Tablets through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of pharmaceutical products for nationwide usage while obtaining volume-based, committed-use pricing. This contract will serve various government participants including VA facilities, Department of Defense (DoD), Indian Health Service (IHS), and Bureau of Prisons (BOP).

Work Details
The contract includes the supply of Metformin HCL 24HR SA Tablets in various packaging sizes as detailed below:

1. **Metformin HCL Tablets, 500mg**
- **30 Tablets per Bottle**: Base Year - 98,898 Bottles; Option Years - 98,898 Bottles each year.
- **60 Tablets per Bottle**: Base Year - 209,377 Bottles; Option Years - 209,377 Bottles each year.
- **90 Tablets per Bottle**: Base Year - 243,836 Bottles; Option Years - 243,836 Bottles each year.
- **120 Tablets per Bottle**: Base Year - 248,223 Bottles; Option Years - 248,223 Bottles each year.
- **180 Tablets per Bottle**: Base Year - 190,545 Bottles; Option Years - 190,545 Bottles each year.
- **360 Tablets per Bottle**: Base Year - 136,897 Bottles; Option Years - 136,897 Bottles each year.
