Background
The Department of Veterans Affairs (VA) is issuing this solicitation to establish a supply source for Hydroxychloroquine SO4 Tablets through its Pharmaceutical Prime Vendor (PPV) Programs. The goal is to ensure availability and consistency of pharmaceutical products for nationwide usage, while obtaining volume-based pricing. The contract will support various government health facilities including VA, DoD, Indian Health Service, and Bureau of Prisons.

Work Details
The contract involves the supply of Hydroxychloroquine SO4 200mg Tablets in two line items:

1. Hydroxychloroquine SO4 200mg Tablet, 100 Tablets per Bottle:
- Base Year Quantity: 163,395 Bottles
- Option Year One Quantity: 163,395 Bottles
- Option Year Two Quantity: 163,395 Bottles
- Option Year Three Quantity: 163,395 Bottles
- Option Year Four Quantity: 163,395 Bottles

2. Hydroxychloroquine SO4 200mg Tablet, 500 Tablets per Bottle:
- Base Year Quantity: 25,853 Bottles
- Option Year One Quantity: 25,853 Bottles
- Option Year Two Quantity: 25,853 Bottles
- Option Year Three Quantity: 25,853 Bottles
- Option Year Four Quantity: 25,853 Bottles. Each offer must include an 11-digit National Drug Code (NDC) number unique to the offeror's company and a Cost Recovery Fee of 0.5%.

Period of Performance
The contract will be effective upon signing and notification of award by the VA. The Implementation Period will last up to a maximum of 60 days after the award date.

Place of Performance
Products will be delivered to various locations as designated by the VA and DoD through their respective Pharmaceutical Prime Vendor distribution centers.
