Background
The Department of Veterans Affairs (VA) Veterans Health Administration (VHA) has a requirement for human blood products and related services. The goal of this contract is to ensure the provision of necessary blood and blood-related products to support the VA's healthcare mission, specifically through the Martinez Outpatient Clinic Transfusion Service. This contract is a Sole Source RFQ to VITALANT for the purchase of these products.

Work Details
The contractor shall provide Human Blood and Blood Related Products as specified in the Statement of Work. The contract includes five ordering periods, each lasting one year, with the following details:

- **Ordering Period 1**: October 1, 2024 – September 30, 2025
- **Ordering Period 2**: October 1, 2025 – September 30, 2026
- **Ordering Period 3**: October 1, 2026 – September 30, 2027
- **Ordering Period 4**: October 1, 2027 – September 30, 2028
- **Ordering Period 5**: October 1, 2028 – September 30, 2029

Specific products include:
- Red Blood Cells (RBCs)
- Fresh Frozen Plasma (FFP)
- Plateletpheresis (non-HLA matched)
- Leukopoor RBCs
- Irradiated Leukopoor RBCs
- Pooled Cryo
- RhIg
- Various testing services including antibody screens and identification.

Quantities are specified per year as follows:
- RBCs: Quantity varies by year.
- FFP: Typically around six units per year.
- Plateletpheresis: One unit per year.
- Additional specific quantities for other blood components as detailed in the price list.

Period of Performance
The period of performance spans five years from October 1, 2024 to September 30, 2029 with specific ordering periods outlined above.

Place of Performance
Products will be delivered to the VA Northern California Healthcare System at the Martinez Outpatient Clinic Transfusion Service located at 150 Muir Road, Martinez CA 94553.
