Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Carbidopa Levodopa Immediate Release (IR) Tablets through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of these pharmaceutical products for nationwide usage, while obtaining volume-based, committed-use pricing. The contract will serve various government participants including VA facilities, Department of Defense (DoD), Indian Health Service (IHS), and Bureau of Prisons (BOP).

Work Details
The contract involves the delivery of Carbidopa Levodopa IR Tablets in various formulations and quantities as follows:

1. **Line Item 1**: Carbidopa 10MG/Levodopa 100MG IR Tablets, 100 Tablets per Bottle
- Base Year: 5,871 Bottles
- Option Year One: 5,871 Bottles
- Option Year Two: 5,871 Bottles
- Option Year Three: 5,871 Bottles
- Option Year Four: 5,871 Bottles

2. **Line Item 2a**: Carbidopa 10MG/Levodopa 100MG IR Tablets, 500 Tablets per Bottle
- Base Year: 4,016 Bottles
- Option Year One: 4,016 Bottles
- Option Year Two: 4,016 Bottles
- Option Year Three: 4,016 Bottles
- Option Year Four: 4,016 Bottles

- Base Year: 2,008 Bottles
- Option Year One: 2,008 Bottles

4. **Line Item 3**: Carbidopa 25MG/Levodopa 100MG IR Tablets, 100 Tablets per Bottle
- Base Year: 97,411 Bottles
- Option Years One to Four: Same quantity each year.
5. **Line Item 4a**: Carbidopa 25MG/Levodopa 100MG IR Tablets, 500 Tablets per Bottle
- Base Year: 118,764 Bottles
6. **Line Item 4b**: Carbidopa 25MG/Levodopa 100MG IR Tablets, 1,000 Tablets per Bottle
7. **Line Item5**: Carbidopa 25MG/Levodopa 250MG IR Tablets, 100 Tablets per Bottle
8. **Line Item6a** : Carbidopa25 MG/Levodopa250 MGIR tablets ,500 tablets per bottle.
9. **Line Item6b** :Carbidopa25 MG/Levodopa250 MGIR tablets ,1 ,000 tablets per bottle.

Period of Performance
The contract will be effective on the date the SF-1449 is signed by the authorized representative and will include a base year with four one-year options.

Place of Performance
Products will be delivered to various locations as specified by the Government Prime Vendor contractors involved in the PPV Programs.
