Background
The Department of Veterans Affairs (VA) is seeking to establish a supply source for Bevacizumab injections through its Pharmaceutical Prime Vendor (PPV) Programs. The goal of this contract is to ensure the availability and consistency of pharmaceutical products for nationwide usage while obtaining volume-based pricing for various government health care facilities, including VA, DoD, Indian Health Service, and Bureau of Prisons. This solicitation aims to create a contract that will provide the drugs listed in the schedule for purchase through these PPV Programs.

Work Details
The contract involves supplying Bevacizumab injections in various formulations and sizes. The following line items are included:

1. **Bevacizumab 25mg/mL, 4mL Injection (Trade name: Avastin)**
- Quantity: 23,448 Vials per year for Base Year and each Option Year.

2. **Bevacizumab 25mg/mL, 16mL Injection (Trade name: Avastin)**
- Quantity: 13,841 Vials per year for Base Year and each Option Year.

3. **Bevacizumab-AWWB 25mg/mL, 4mL Injection (Trade name: Mvasi)**
- Quantity: 23,448 Vials per year for Base Year and each Option Year.

4. **Bevacizumab-AWWB 25mg/mL, 16mL Injection (Trade name: Mvasi)**
- Quantity: 13,841 Vials per year for Base Year and each Option Year.

5. **Bevacizumab-MALY 25mg/mL, 4mL Injection (Trade name: Alymsys)**
- Quantity: 23,448 Vials per year for Base Year and each Option Year.

6. **Bevacizumab-MALY 25mg/mL, 16mL Injection (Trade name: Alymsys)**
- Quantity: 13,841 Vials per year for Base Year and each Option Year.

7. **Bevacizumab-ADCD 25mg/mL, 4mL Injection (Trade name: Vegzelma)**
- Quantity: 23,448 Vials per year for Base Year and each Option Year.

8. **Bevacizumab-ADCD 25mg/mL, 16mL Injection (Trade name: Vegzelma)**
- Quantity: 13,841 Vials per year for Base Year and each Option Year.

9. **Bevacizumab-BVZR 25mg/mL, 4mL Injection (Trade name: Zirabev)**
- Quantity: 23,448 Vials per year for Base Year and each Option Year.

10. **Bevacizumab-BVZR 25mg/mL, 16mL Injection (Trade name: Zirabev)**
- Quantity: 13,841 Vials per year for Base Year and each Option Year.

Offerors must submit prices that include a Cost Recovery Fee of 0.5% and provide an eleven-digit National Drug Code (NDC) number unique to their company for each drug offered.

Period of Performance
The contract will be effective on the date the VA signs the SF-1449 form and will include a base year with four one-year options.

Place of Performance
The products will be delivered to various Department of Veterans Affairs facilities as well as other authorized users under the Department of Defense Pharmaceutical Prime Vendor Program.
