Description

Enclosed is a Request for Quotations (RFQ) for to provide maintenance services for the sanitary sewer collection system and the wastewater treatment system at the U.S. Consulate, Surabaya, Indonesia. If you would like to submit quotation, follow the instructions in Section 3 of the solicitation, complete the required portions of the attached document, and submit it to the address shown on the Standard Form 1449 that follows this letter.

The U.S. Government intends to award a contract/purchase order to the responsible company submitting an acceptable offer at the lowest price. We intend to award a contract/purchase order based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.

The U.S. Consulate General Surabaya intends to conduct a pre-quotation conference at the site. The pre-proposal conference will be held on April 24, 2025, at 09:00 (local time) at the U.S. Consulate General Surabaya. Prospective offerors/quoters should contact marjoko@state.gov by April 22, 2025, at 10:00 for additional information or to arrange entry to the building.

Quotations are due by May 16, 2025, at 16:00. No quotations will be accepted after this time. Proposals must be in English and incomplete proposals will not be accepted.

Your quotation must be submitted electronically to marjoko@state.gov. It is important to make sure the submission is made in specific size and format; in MS-Word 2007/2010 or MS-Excel 2007/2010 or Adobe Acrobat (pdf) file format. The file size must not exceed 30MB. If the file size should exceed the 30MB, the submission must be made in separate files and attached to separate emails with less than 30MB each.

Work Details
The contractor will provide comprehensive maintenance services for both the sanitary sewer collection system and the wastewater treatment system. Key tasks include:

1. Performing all maintenance services as outlined in the scope of work, ensuring no more than one customer complaint is received per month.
2. Conducting preventive maintenance visits semi-annually, with each visit lasting a minimum of five consecutive days.
3. Providing necessary chemicals and materials for wastewater treatment, including liquid bacteria and nutrients for bacteria growth.
4. Conducting routine operator testing of the systems and providing technical analysis based on results submitted by Consulate staff.
5. Reviewing daily operating logs during site visits to verify safe operation and providing instruction on correcting deficiencies.
6. Ensuring compliance with various safety standards and regulations, including OSHA, NFPA, and local health codes.
7. Submitting a written service report within ten business days after each visit detailing test results, comments on system status, and recommendations for action if necessary.

Period of Performance
The contract will be for a one-year period starting no later than June 2025, with options for four additional one-year periods.
