The objectives of this collaborative agreement is to provide technical assistance to HRSA grantees and other providers of healthcare services by supporting the Healthcare Communities Knowledge Gateway, a robust knowledge and content management system portal, and related quality improvement efforts that bridge healthcare, public health, local and state based community organizations in improvement efforts across the United States. Healthcare Communities Knowledge Gateway provides support to the following: (a) Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) participant organizations and related community based activities; (b) the Donation and Transplantation Community of Practice (DTCP) continued growth and success in saving lives and increasing awareness of the need for additional donors; and (c) information resources for the State Health Access program.

B - Cooperative Agreements (Discretionary Grants)

Public, private, for profit and not-for-profit entities are eligible to apply. Federally recognized Indian Tribal Government and Native American Organizations are eligible to apply. The applicant must have the ability to provide technical support and development; national project management of community based improvements, results sharing with automated analysis, and associated resources; and improve community resources including payer and health information technology systems.

Beneficiaries include current users such as eligible organizations and a variety of HRSA grantees including but not limited to: health centers, Counties and States, transplant hospitals and centers, qualified organ procurement organizations, educational institutions, state and local health departments, public health and other private entities.
