93.426: Improving the Health of Americans through Prevention and Management of Diabetes and Heart Disease and Stroke
Alternate Name: The purpose of this program is to implement and evaluate evidence-based strategies to address the challenges and systemic barriers that contribute to prevention and management of cardiovascular disease and diabetes in high-burden populations. The Selection of high burden populations will be based on a robust analysis of diabetes and cardiovascular health burden across geographic areas and population subgroups. Population subgroups may be defined by factors such as race or ethnicity, gender, age [e.g. youth and older adults], education, income, disability, or sexual orientation, among others.
