Objective
The Center for Medicare and Medicaid Innovation (CMMI) is releasing a Notice of Funding Opportunity announcement for the ET3 model – a new, voluntary, five-year payment model that will provide greater flexibility to ambulance care teams to address emergency health care needs of Medicare Fee-for-Service (FFS) beneficiaries following a 911 call. This announcement will present the funding opportunity for state and local governments, their designees, or other entities that operate or have oversight over a Public Safety Answering Point (PSAP). The intervention for this funding will be to establish or expand on a medical triage line integrated with the PSAP, with the goal of reducing inappropriate use of emergency ambulance services and increasing efficiency in Emergency Medical Services (EMS) systems. In geographic locations where ambulance suppliers and providers have been selected to participate in the ET3 model, eligible applicants will have the option to apply for this cooperative agreement aimed at allowing existing PSAPs to redirect callers with non-emergency conditions to appropriate care alternatives.

Applicant Eligibility
An entity must meet all of the following criteria in order to be eligible to apply to this NOFO:
• Is a state or local government entity or its designee, or another entity that operates or has authority over a PSAP.
• Has the authority to implement a medical triage line in its geographic region
• Proposes to implement or expand a medical triage line in a geographic region that includes a model participant’s model region
• Operates or has authority over the PSAP that receives 911 Calls in a geographic region that includes a model participant’s model region as well as the authority to implement the triage intervention of the ET3 model integrated with the PSAP.
Beneficiary Eligibility
Applicants eligible to apply to this NOFO must operate or have authority over a PSAP in the same geographic region as an ET3 model participant (i.e., an ambulance provider or supplier in the ET3 model).
