The purpose of this five-year funding opportunity is to provide funding, training, technical assistance and consultation on evidence-based interventions and strategies, surveillance and epidemiology, and program evaluation designed to build or strengthen capacity. This funding opportunity offers funding to support tribal tobacco control programs to reduce commercial tobacco use and abuse among American Indian (AI) and Alaska Native (AN) tribal members in order to achieve the goal of the NCCDPHP to reduce chronic diseases and promote health, using the framework of the four domains. The NCCDPHP four domains consist of core focus areas that will transform the nation’s health and provide Americans with equitable opportunities to take charge of their health and are as follows: (1) Epidemiology and Surveillance; (2) Environmental Approaches that Promote Health and Support and Reinforce Healthful Behaviors; (3) Health System Interventions; and (4) Community-Clinical Links.

B - Cooperative Agreements

Eligibility is limited to federally recognized American Indian/Alaska Native tribal governments, American Indian/Alaska Native tribally designated organizations, Alaska Native health corporations, Urban Indian health programs and Tribal Epidemiology Centers. These entities have received direct funds to build capacity for tribes to conduct evidence-based tobacco control interventions for the past 14 years. Prior to this, state public health programs were encouraged to collaborate with tribes and tribal organizations within their jurisdiction. Federally recognized American Indian/Alaska Native tribal governments, American Indian/Alaska Native tribally designated organizations, Alaska Native health corporations, Urban Indian health programs and Tribal Epidemiology Centers are uniquely qualified to build infrastructure and capacity for tribes to conduct evidence-based tobacco control interventions. These entities are able to: • Implement effective, culturally appropriate improvements in community environments, provide community-clinical linkages; and improve health care systems • Integrate evidence-based practices to reduce commercial tobacco prevalence rates in tribal populations and are able to make a significant impact on individual and community health of American Indians and Alaska Natives • Carry out the intent of the proposed project Other organizations, including state and local health departments, do not have the capacity to reach multiple tribes, which is the intent of this cooperative agreement, nor do they have the capacity to inform, educate, and advocate on behalf of AI/AN populations.
