Background
The Texas State Board of Pharmacy (TSBP) is issuing this solicitation for the production and distribution of Official CII Prescription Forms required by Texas practitioners for prescribing controlled substances. This contract aims to ensure compliance with the Texas Health and Safety Code, which mandates that controlled substances be prescribed using approved forms.

Work Details
The contractor will be responsible for producing and distributing TSBP-approved CII Prescription Forms to authorized prescribers. The forms must meet all specifications set forth by the TSBP, including compliance with applicable regulations and standards.

All items provided must be new, unused, and of current production. The contractor must also ensure that all electrical items meet OSHA standards and bear appropriate listings from UL, FMRC, or NEMA.

Period of Performance
The contract will be performed over a period specified in the solicitation, typically lasting several months to a year, with potential extensions at the discretion of the TSBP.

Place of Performance
The products will be delivered to designated locations as specified by the Texas State Board of Pharmacy.

Bidder Requirements
Respondents must comply with all purchasing rules and regulations in Texas. They must provide their Texas Identification Number (TINS) and certify that they are not ineligible to receive state contracts.

Additionally, respondents are required to submit a HUB Subcontracting Plan (HSP) if subcontracting opportunities are probable under this contract.
