Health Care Employee Assistance Program
Background
The State of Montana, State Procurement Services Division, on behalf of the Montana Health Care and Benefit's Division, is issuing this Request for Proposal (RFP) to provide Employee Assistance Program Services. The goal of the contract is to procure EAP services for the Health Care Employee Assistance Program.

Work Details
The required services include providing Employee Assistance Program (EAP) Services for a 3-year period. The contractor needs to enter the combined 3-year total for EAP Services for PEPM in the Cost Proposal Document. The services should be provided as a Lump Sum delivery.
