Grant Description
Ltbb Sdpi Fy 2023 - The Little Traverse Bay Bands of Odawa Indians' 60-month Special Diabetes Program for Indians (SDPI) will promote diabetes education, care and prevention through the IHS best practice of diabetes-related education.
The required key measure is the number and percent of individuals in our target group who receive education on any diabetes topic, either in a group or individual setting.
Diabetes-related education activities include self-guided wellness plans, lifestyle education, nutrition education, physical activity education, community events, and the Home-Grown Project.
Other activities/services not related to our best practice are use of traditional healers and youth-based diabetes-related education.
