PART 1.0 – SCOPE OF WORK:

1.1 GENERAL: The work to be performed under this contract and in accordance with this

Statement of Work shall consist of furnishing all necessary parts, labor, tools, transportation,
supplies, supervision, equipment, materials, and incidentals necessary for providing all work
shown on the drawings (Technical Exhibits), statement of work, and all applicable codes,
regulations, standards and criteria in effect at the date of solicitation. The work outlined
below shall consist of, but not be limited to the following:

BACKGROUND: Buildings 140, Mathis Gymnasium, located at 155 W. Texan St. on
Goodfellow Air Force Base, has experienced a concrete floor/slab deficiency/failure within
the building envelope, specifically the East perimeter wall of rm. 113, and both the North and
East walls of rm. 114. The deficiency/failure of the concrete floor/slab has settled as much as
one (1”) inch in some areas and can be visually seen. Upon review of archived construction
drawings, the concrete floor/slab appears to be constructed as a floating concrete floor/slab.
Sheet S2, Section 14, (TE-6), depicts a 6” concrete floor/slab, vapor barrier, 8” capillary water
barrier, and 24” minimum non-expansive fill. The Government has had both an elevation
survey, (TE-7&8), and geotechnical service performed and has received a geotechnical report
from the provider. (TE-9 Geotechnical report.)
It is the intent of the Government to have the concrete floor/slab lifted back to as close to
as originally installed.

1.2 PROJECT DESCRIPTION: Provide deep injection soil stabilization of base and sub-base
soils under an existing concrete slab by filling voids, undersealing, raising, and support the
existing concrete slab by drilling injection holes, furnishing, and injecting a water resistant
“High Density Polyurethane Foam (HDPF) into the foundation soils and injecting/pumping a
polyurethane material under the slab and into the base and sub-base soils through the drilled
holes, while monitoring the movement at the surface in rooms 113 and 114 of B140 Mathis
Gymnasium.
The Contractor shall, 30 days after completion of the foam injection, perform testing and
provide test reports to ensure the soil stabilization has been achieved. Testing should include,
but not limited, soil moisture content, Atterberg Limits, sieve analysis, and allowable bearing
capacity values. The report shall include site pictures, site conditions, and site map, Potential

Vertical Rise estimates, as well as results from each test performed. The testing shall be
performed by a third party entity that performs this type of testing. After slab lifting has been
completed the Contractor shall install new rubber resilient sheet flooring and rubber cove base.

1.3 LOCATION: Goodfellow Air Force Base is located in Tom Green County, on the southeast
side of San Angelo, TX. and is bounded to the north by Highway #388 (Paint Rock Rd.), to
the west by Fort McKavitt Rd and Bell Street/Christoval Road, to the south by Highway #1223
(San Antonio Hwy.) and to the east by the eastern city limits. Work location is in Building
140, Mathis Gym, 155 W. Texan St. Building 140 hours of operation are Monday – Friday,
4:30am – 9:00pm, Saturday and Sunday, 10:00am – 6:00pm.

1.4 SITE VISITS: The Contractor is encouraged too and may visit the premises to become

thoroughly familiar with details of the work and working conditions, verify dimensions in
the field, and shall advise the Contracting Officer of any discrepancies before starting the
work.

1.5 WORK AND MECHANICS: The work for this project shall be executed in the best and

most workmanlike manner, by qualified and efficient mechanics/tradesmen, skilled in their
respective trades. The Polyurethane Contractor must be regularly engaged in work similar to
this project in difficulty and/or scope. Only certified journeymen in each respective trade, or
apprentices under the direct supervision of certified journeymen, shall be permitted to install
and/or supervise installation for this project. Individual trade work for this project shall be
performed and quality maintained by the applicable trade, only. All trades shall coordinate
their work with that of other trades. The Contractor shall coordinate and perform all
operations in a manner that will result in a professional and expeditiously completed project.
The work shall be in strict accordance with prevailing industry standards and manufacturer’s
instructions. Work and materials shall comply with this Statement of Work and the editions
in effect at the time of this solicitation for all applicable criteria, regulations, guidelines and
codes, all of which are made a part thereof.

1.6 PERFORMANCE PERIOD: The Contractor shall have forty-five (45) calendar days to
perform all work associated with this project. The Contractor shall submit product
specifications for all new materials within fourteen (14) calendar days before signing the
NTP. The Contracting Officer must approve all materials before the Contractor makes any
purchase.

All Contractor operated/owned vehicles, equipment, tools, etc., shall be stored as prescribed
in paragraph 1.16: DELIVERY and STORAGE. Applicable traffic control signage where
required by the contract shall remain overnight. Parking on turf shall not be permitted
without prior approval by the Government.

1.7 WORK SCHEDULE: Normal work days for Goodfellow AFB are from 0730 to 1630,
