CBOC Services - Alamogordo, NM for New Mexico VA Health Care System (NMVAHCS)   2

Background
The Department of Veterans Affairs New Mexico VA Health Care System (NMVAHCS) is seeking to provide essential Primary Care and Mental Health Services for Veterans in Alamogordo, New Mexico.
The goal of this contract is to ensure that veterans receive comprehensive healthcare services in a Community-Based Outpatient Clinic setting, aligning with the VA's mission to improve the health and well-being of veterans.

Work Details
The contractor will be responsible for delivering a range of services including but not limited to:
1. Providing integrated, accessible health care services by clinicians who are accountable for addressing a majority of personal health care needs.
2. Conducting diagnosis and management of acute and chronic biopsychosocial conditions, health promotion, disease prevention, overall care management, and patient and caregiver education.
3. Staffing requirements include a Patient Aligned Care Team (PACT) consisting of Primary Care Providers (PCPs), Registered Nurse Care Managers (RNCMs), Clinical Associates, Administrative Associates, and other discipline-specific team members such as Clinical Pharmacist Practitioners (CPPs) and Licensed Social Workers.
4. Ensuring continuity of care during periods of inadequate resources or staff turnover by implementing contingency plans.
5. Compliance with VA standards for patient panel sizes and staffing ratios, ensuring that each primary care provider has a manageable caseload.
