Background
This modification is issued to update the NAICS code from 561330 to 621399. All other conditions remain the same. The contractor shall provide all personnel, equipment, tools, materials, supervision, and quality control necessary to provide Medical Readiness and Case Management support to include; Medical Operations Program Manager support, Administrative Specialist support, part-time Audiologist support, Medical and Behavioral Health Case Management Services, and Medical Records Support.

Work Details
The contractor shall provide Medical Readiness and Case Management support for the Georgia Army National Guard (GA ARNG). This includes maintaining Department of Defense established goals for overall Medical Readiness Classification (MRC) 1 and 2, MRC 3 at less than 8%, and MRC 4 at less than 2%. The contractor shall facilitate monthly meetings with the COR to ensure proper management of all medical cases, track all MRC 3 and MRC 4 soldiers on a case tracker report, consolidate and submit weekly medical readiness data of medical and dental readiness categories, facilitate PHA events for SMs to determine their overall deployability status, and conduct intake services at PHA and Medical Readiness.
Additionally, the contractor shall provide Administrative Specialist Services including preparing reports using data from the ARNG Patient Tracking Report, coordinating care received by TRICARE and by the Defense Health Agency-Great Lakes (DHA-GL), documenting medical care plans bi-monthly in eCase, performing hearing evaluations for GA ARNG Service Members who are pre-identified as H3 or testing currently as H3 according to AR 40-501 and DA PAM 40-501.

Period of Performance
The period of performance will begin on Sep 06, 2023 and last until the established period of performance end date of Sep 06, 2024. Period of performance extensions beyond this established end date will only be granted via fully executed supplemental agreement signed and issued by the contracting officer.

Place of Performance
The place of performance will be co-located with each Major Subordinate Command (MSC) Headquarters or as assigned throughout the State at one of the aforementioned locations including Clay National Guard Center (CNGC) in Marietta, Fort Stewart, Columbus GAARNG Element in Columbus GA.
