The solicitation W912PQ25QA013 is issued as a Request for Quote (RFQ) from the USP&FO for New York, Purchasing and Contracting Division, Latham, New York a Firm Fixed Price (FFP) indefinite delivery/indefinite quantity (IDIQ) contract for Audiology Services throughout New York State (NYS) as defined in the Performance Work Statement (PWS) for NYARNG MEDCOM.

Principle NAICS is 621340 and size standard is $12,500,000.00. This acquisition is utilizing procedures in FAR part 12 and FAR part 13.5 Simplified Acquisition Procedures. This requirement is unrestricted.
52.204-26 MUST be included with the quote, if not in sam.gov registration.

All questions/RFI's must be submitted in writing via an email to johanna.a.goman.civ@army.mil and melissa.santoro2.civ@army.mil no later than 18 July 2025 at 9:00 am ET.

All quotes must be submitted by email to johanna.a.goman.civ@army.mil and melissa.santoro2.civ@army.mil by 30 July 2025 12:00pm ET.

These quotes shall conform to the requirements of 52.212-1 listed in the Solicitation Document under Addendum to 52.212-1, Instruction to Offerors. The Government will not accept partial quotes/offers. Award will be made without discussions.

Award will be made based on price and other factors as shown below. All pricing must be provided for, this requirement will not be split. If something left blank proposal will not be considered. See Addendum to 52.212-2 Evaluation- Commercial Items

Wage Determination 2015-4143 Revision: 32 dated 08 July 2025 applies and is attached.

In order to be eligible for award, the successful offeror's System for Award Management (SAM) (https://www.sam.gov) registration must be active and includes representations and certifications.
