The purpose for this amendment is to revise Attachment 1 Pricing Worksheet and to Attachment 11 to add wage determination 2015-4945 Rev 6 and 2015-4471 Rev 8.
PLEASE NOTE: The revised Attachment 1 (REV 1) dated 03232018 must be submitted with the final proposal, all other previous versions are obsolete.

All other terms and conditions remain unchanged.

This is a commercial Personal Services requirement under FAR Part 12 Acuisition of Commerical Items, as supplemented.

Set-aside status: No Set-Aside

This requirmeent is to establish an Indeefinite Delivery Indefinite Quantity (IDIQ) contract for Federal Occupational Health (FOH)

This requirement is to procure a single IDIQ Labor Hour contract to provide professional medical staffing, occupational health professionals and ancillary staff to support the Department of Health and Human Services (HHS) Program Support Center (PSC) Federal Occupational Health (FOH) - National Occupational Health Support Services Contract (NSC).

This is a controlled/unclassified posting for the RFP and attachments and as such explicit access must be provided. All requests for access need to be submitted through FBO.
