ACL - NP, UC and PC Physicians

Background
The Albuquerque Area Indian Health Services, specifically the Acoma Canoncito Laguna Indian Health Center, is seeking to fulfill a critical staffing requirement for healthcare services. The agency's mission is to provide comprehensive health services to the American Indian population in the region. The goal of this contract is to secure two Nurse Practitioners, one Primary Care Physician, and one Urgent Care Physician to ensure adequate healthcare delivery at their facility located at 80B Veterans Blvd., Acoma, New Mexico 87034.

Work Details
The contractor will provide the following services:
1. Two Nurse Practitioners (NP)
2. One Primary Care Physician (PCP)
3. One Urgent Care Physician (UCP)

The services include but are not limited to:
- Conducting medical history assessments and physical examinations.
- Diagnosing medical conditions across all age groups from pediatric to geriatric patients.
- Ordering laboratory tests and x-rays as necessary.
- Referring patients to outside specialists when required.
- Coordinating patient referrals through the Purchase Referred Care office.
- Maintaining accurate Electronic Health Record (EHR) documentation within 24 hours of patient visits.
- Prescribing medications and treatment therapies while collaborating with nursing and pharmacy staff.
- Providing individual counseling for social and emotional issues and recognizing when referrals to mental health professionals are necessary.

Period of Performance
The contract will have a base year from November 1, 2024, through October 31, 2025, with options for three additional years extending through October 31, 2029.

Place of Performance
The contract will be performed at the Acoma Canoncito Laguna Indian Health Center located at 80B Veterans Blvd., Acoma, New Mexico 87034.
